The GP Liaison Unit assists General Practitioners (GPs) to navigate the services provided by St Vincent’s Hospital. The team also works to improve communication between the hospital and general practice.

- It can be requested from Health Information Services (HIS) via the form below, or practice letterhead including the required information.

National Health Services Directory (NHSD) - Health Direct
Maintaining up to date provider details in the National Health Services Directory (NHSD) – Health Direct is critical to receiving timely patient information as public health services use this database to obtain GP/practice contacts.
Please include individual GP names and provider numbers. Provider numbers are not publically listed on the NHSD website.

Navigational Tool - Community and @Home Services
This tool is designed to help referring clinicians navigate the expanding range of Community and @Home services on offer, and identify the most appropriate service(s) for their patients.
Statewide Referral Criteria
The Victorian Department of Health has worked with stakeholders to develop state-wide referral criteria (SRC) to improve the access to specialist clinics in public hospitals by improving the quality and appropriateness of referrals.
SRC exist for a range of medical and surgical presentations and conditions and can be found at: https://src.health.vic.gov.au/specialities
SRC are incorporated in to HealthPathways advice, where applicable.
HealthPathways Melbourne
HealthPathways provides clear and concise guidance for assessing and managing a patient with a particular symptom or condition in primary care. It includes information about making referrals to services in the local health system.
