Residential In-Reach Service (RIR)

Residential Aged Care In-Reach provides hospital type care, where appropriate to people living in aged care facilities (RACF). RIR is staffed by Clinical Nurse Consultants, Nurse Practitioner Candidates and Doctors from the Geriatric Medical Team. The RIR visit residents in aged care facilities providing specialist assessment and treatment within the aged care facility. This may help prevent an emergency department presentation for the frail older resident. RIR is not intended to replace the care the resident receives from their usual doctor.

Our Services

We can provide clinical assistance with but not limited to;

Comprehensive Medical and Nursing Assessment, Medication Reviews, Advance care planning and family meetings, Palliative care advice and referral, Intravenous Antibiotic treatment for chest infections and Urinary Tract infections, the confused resident advice assessment and referral, pain assessment and management

Service Delivery

7 Days per week service

Mon-Friday 0800-1800hrs

Sat Sun and Public Holidays 0800-1600hrs

Eligibility criteria

SVHM Primary Catchment: Any resident from an aged care facility in the City of Yarra, City of Boroondara and southern part of City of Darebin.

Referral Process

We accept referrals from staff of Aged Care Facilities, General Practitioners
