Interdisciplinary program for people who have diagnosed heart and/or lung conditions (including N/STEMI, PCI/stent, CAGS, heart failure (all types), COPD, ILD, bronchiectasis, chronic asthma, etc). Offers self-management education, exercise and motivational support to improve physical and psychosocial function and quality of life. Team members Allied health assistant Nurse Physiotherapist With input from Dietitians, Occupational Therapists and Social Workers. Service delivery We offer group classes at Kew and Fairfield for the following: Cardiac Rehab - Eight-week program Heart failure - 10-week program Pulmonary failure - 10-week program Long COVID - 10-week program Eligibility criteria Diagnosed cardiopulmonary conditions by any medical professional. Willingness to participate in service and open to learning new health and lifestyle strategies to improve own health.
