Post Acute Care (PAC)

St Vincent’s receives Post Acute Care services through Inner Melbourne Post Acute Care: a service delivered by North Richmond Community Health. 

Inner Melbourne Post Acute Care (IMPAC) is a state government funded program that provides short-term services to help people recover at home following a stay in a public hospital.


Post Acute Care aims to:

  • support healing and recovery
  • fill the gap until longer-term services commence
  • reduce the risk of re-admission to hospital
  • provide a rapid response
  • provide a flexible service that responds to each client’s particular needs.

A variety of services can be arranged and funded. Some of the most common services arranged are:

  • Home care
  • Personal care
  • Shopping assistance
  • Nursing
  • Physiotherapy
  • Industrial cleaning
  • Respite or emergency accommodation

How does Inner Melbourne Post Acute Care work?

  • Referrals can be made any time during office hours.
  • Care Coordinators meet each morning to review and allocate new referrals.
  • New urgent referrals received are actioned during the day.

The Care Coordinator will:

  • Telephone the client and their family to discuss and arrange services.
  • Purchase the services needed through contracted providers.
  • Liaise with any community service agencies who are involved with the client’s care.
  • Monitor service and care needs by regular phone contact with the client, their family and other involved agencies.
  • Liaise with agencies that will provide longer term services, for example local councils.
  • Make referrals to other community agencies if needed.
  • Feedback and follow up with the hospital and GP if needed.
