Community and Aged Care

Aged and Community Care at St Vincent's consists of: Health Independence Program (HIP), Home Based Allied Health Service, Hospital in the Home (St Vincent's at Home), Aged Care Assessment Service (ACAS), Aged Psychiatry Assessment and Treatment Team (APATT), Geriatric Evaluation and Management (GEM) and our Aged Care Facilities.


Specialist Clinics for Older Persons

The Specialist Clinics at St George’s Hospital include the Continence Clinic, Cognitive Dementia and Memory Service (CDAMS), Geriatric Medical Clinic, and Falls and Balance Assessment Clinic

Aged Care Assessment Service (ACAS)

ACAS

My Aged Care provides a free Aged Care Assessment Service (ACAS) if a patient is considering moving into an aged care home or requires help at home.

Health Independence Program (HIP)

HIP was introduced by the DH&HS in 2008 bringing together the four previously separate service streams of Subacute Ambulatory Care, Post Acute Care, Hospital Admission Risk Program and Residential In Reach under common guidelines.

Geriatric Evaluation and Management (GEM)

St Vincent’s has Geriatric Evaluation and Management units at Fitzroy and St George's in Kew. The Fitzroy GEM Unit comprises 22 beds and is located on the 3rd floor of Building F - Bolte Wing. The St George’s GEM Unit comprises 30 beds and is located on the first floor of St George’s.

Home Based Allied Health Services (Domiciliary/Rescare)

The Domiciliary Allied Health Therapy Team is a Home and Community Care (HACC) funded program providing allied health interventions including physiotherapy, occupational therapy, speech pathology, podiatry, and dietetics to frail elderly or younger disabled clients who are housebound.

Aged Psychiatry Assessment and Treatment Team (APATT)

APATT

St Vincent's Aged Psychiatry Assessment and Treatment Team (APATT) is a multidisciplinary community mental health service based at St George’s campus in Kew. Referrals from GPs are welcomed.

Transition Care Program

The Transition Care Program (TCP) is a Commonwealth and State funded program. The St Vincent's TCP team is multidisciplinary. Clients can be referred immediately after a hopsital stay, they cannot be referred from community.

St Vincent's Hospital Melbourne's has three aged care facilities - Auburn House in Hawthorn and Prague House and Berengarra in Kew.
