The GP Liaison Unit assists General Practitioners (GPs) to navigate the services provided by St Vincent’s Hospital. The team also works to improve communication between the hospital and general practice.

Maintaining up to date provider details in the National Health Services Directory (NHSD) – Health Direct is critical to receiving timely patient information as public health services use this database to obtain GP/practice contacts. Please include individual GP names and provider numbers. Provider numbers are not publically listed on the NHSD website.

Statewide Referral Criteria

The Victorian Department of Health has worked with stakeholders to develop state-wide referral criteria (SRC) to improve the access to specialist clinics in public hospitals by improving the quality and appropriateness of referrals. SRC exist for a range of medical and surgical presentations and conditions and can be found at: https://src.health.vic.gov.au/specialities SRC are incorporated in to HealthPathways advice, where applicable.
