Complex Aged Care (TRAAC)

Treatment, Response and Assessment for Aged Care (TRAAC) is a St Vincent's Hospital Admission Risk Program (HARP) service for older people with complex needs. TRAAC comprises a multidisciplinary team of clinicians who are specialised in aged care. TRAAC clinicians come from various backgrounds including nursing and allied health.

TRAAC offers services to clients of St Vincent's who are experiencing a change in health or functional status and are at risk of presenting to hospital. 

TRAAC clients are generally 65 years old and over, and:

  • Have complex medical conditions            
  • Have declining health in past 3 months e.g. changes in memory or falls
  • Are unable to manage their health
  • Need more support or services due to carer stress or illness

About Us

TRAAC clinicians visit clients in their home and provide rapid response assessment and short term care coordination. A TRAAC clinician also works closely with HARP specialist clinicians including occupational therapists, psychologists, speech pathologists and pharmacists, and coordinates assessment and care planning.

TRAAC aims to provide:

  • Comprehensive home based aged care assessment
  • Short-term care coordination
  • Immediate and future care planning for the client and their carer
  • Referral and coordination with community services such as personal care, home help, meals on wheels and nursing needs
  • Short-term, flexible service increase unable to be met by mainstream services
  • Rapid access to Geriatrician and Allied Health services including Psychology, Physiotherapy, Occupational Therapy and Speech Pathology
