Aged Care Assessment Service (ACAS)

My Aged Care provides a free Aged Care Assessment Service (ACAS) if a patient is considering moving into an aged care home or requires help at home.

The role of ACAS is to determine the overall care needs of frail older people (over 65 years of age) and to assist them to gain access to the most appropriate types of services.

In doing so, ACAS aims to:

  • Comprehensively assess older people taking their restorative, physical, medical, psychological, cultural and social needs into account
  • Involve clients, their carers and service providers in the assessment and care planning process
  • Provide approval to access case managed packages of care for people with complex needs
  • Provide approval for Commonwealth funded aged residential and respite care.

 
GPs are particularly encouraged to contribute to the planning process and ACAS welcomes their referrals, participation and input.
